- WHERE WE GET PERSONAL INFORMATION FROM
We obtain your details when you enquire about our care and service, when you join the practice, when you subscribe to our newsletter or register online, when you complete a registration or medical history form, from family members or carers, and when another healthcare provider refers you for treatment at our practice.
We will be using CCTV to:
- Protect the practice premises and property
- Increase the safety of patients, staff and visitors
- Deter criminal activity and anti-social behaviour
- Assist in the apprehension, identification and prosecution of offenders (if required)
- Provide evidence to a court or tribunal (if required)
- HOW LONG WE KEEP INFORMATION
We minimise the data that we keep, and do not keep it for longer than necessary.
We keep your records for 11 years after the date of your last visit to the Practice or until you reach the age of 25, whichever is longer. At your request, we will delete non-essential information (for example, some contact details) before the end of this period.
Images captured with CCTV will be kept and destroyed securely after 3 months unless they are needed for longer due to ongoing investigation, legal proceedings, or regulatory compliance.
- HOW WE STORE INFORMATION
We store your personal information securely on our practice computer system and in a manual filing system. Only those working at the practice have access to your information. They understand their legal responsibility to maintain confidentiality and follow practice procedures to ensure this.
We take precautions to ensure the security of the practice premises, the practice filing systems and computers.
We use high-quality specialist dental software to record and use your personal information safely and effectively. Our computer system has a secure audit trail, and we back up information routinely.
- SHARING INFORMATION
To provide you with appropriate care, we might need to share personal data with the following;
however, only the minimum information required will be shared:
- Another dentist or other health professional who is caring for you
- Your GP, consultant, or Occupational Health Professional
- A laboratory
- The Inland Revenue
- A private dental scheme, if you are a member

- SHARING CCTV RECORDINGS
When using or processing CCTV recordings, we will respect the legal rights of the individuals shown in them. We will not share images or recordings except in the following circumstances:
- If requested by the local authority, police or courts for the investigation, prevention or prosecution of anti-social behaviour or criminal activity
- For bringing or defending a legal claim
- To comply with a police warrant or an order given by a court or tribunal

We obtain your details from you directly, your legal representatives, and third parties, such as referees, employment agencies, occupational health providers, pension administrators, government departments (e.g. HMRC, DWP), and public sources (e.g. LinkedIn).
We do not currently use CCTV. However will be implementing this to:
- Protect the practice premises and property
- Increase the safety of patients, staff and visitors
- Deter criminal activity and anti-social behaviour
- Assist in the apprehension, identification and prosecution of offenders (if required)
- Provide evidence to a court or tribunal (if required)
- HOW LONG WE KEEP INFORMATION
We minimise the data that we keep, and do not keep it for longer than necessary.
Criminal record check information is not kept for longer than is necessary—usually not more than 6 months following the recruitment decision, unless a dispute is raised or, in exceptional circumstances.
The retention period for staff records is 6 years; however, certain information relating to team members may lawfully be kept for longer. Team members should refer to the Record Retention Schedule (M 215A) for further details. Application forms and interview notes for unsuccessful candidates are kept for one year.

We maintain electronic and paper records related to your recruitment and employment. The practice holds this information with your line manager. All paper files are securely stored, and only relevant staff can access them.

For tax purposes the law requires us to keep basic information about our customers (including Contact, Identity, Financial and Transaction Data) for six years after they stop being customers.

Dr Marcello Tulone is responsible for keeping secure the information about you that we hold.

Those at the practice who have access to your information include dentists and other dental professionals involved with your care and treatment, and the reception staff responsible for the management and administration of the practice.

To provide you with the dental care and treatment that you need, we require up-to-date and accurate information about you.
We will share your information with NHS England in connection with your dental treatment.
We will seek your preference for how we contact you about your dental care. Our usual methods are telephone, email or SMS.
If we wish to use your information for dental research or dental education, we will discuss this with you and seek your consent. Depending on the purpose and if possible, we will anonymise your information. If this is not possible we will inform you and discuss your options.
We may use your contact details to inform you of products and services available at our Practice. This may include short notice appointment availability, our newsletters and offers we are running.

We use cloud computing facilities for storing some of your information. The practice has a rigorous agreement with our provider to ensure that we meet the obligations described in this policy and that we keep your information securely.
We keep your records for 10 years after the date of your last visit to the Practice or until you reach the age of 25 years, whichever is the longer.
Access to your information and other rights
You have a right to access the information that we hold about you and to receive a copy. You should submit your request to the practice in writing or by email. We do not usually charge you for copies of your information; if we pass on a charge, we will explain the reasons.
You can also request us to
